Ready to become a patient?

We thank you for considering us as your Primary Care Home! If you would like to register as a new patient, please follow these instructions:

-Select the "Documents" tab

​

-Download and complete the New Patient Packet

​

-Return the packet by email to info@canyonfamilyhealth.com, fax to (503)767-3227, drop it off in person or mail it to us.

​

We ask for 72 business hours to review your packet and will reach out to you by phone.

ACCEPTED INSURANCE

We work with a wide variety of insurance carriers, however coverage varies depending on your company and plan.
